To register for our Patient Online Service you will need to complete and submit this form.
Please note once you have submitted the form we will need to you to check personal identification and we need to see a proof of address before we can set you up on online service. You will need to come into the Surgery in person within 7 working days, with one form of photo ID such as valid passport or drivers’ licence and with a proof of address, such as a utility bill addressed to yourself and dated within the last three months.
We will not be able to process your registration will not be processed until you have provided the required documents. If we have not received the requirements within 7 days, your request will be deleted from the system and you will need to fill in an complete another registration form.
Once we have completed your registration we will then issue you a username and password.
Once you are registered you will be able to use the Patient Online Service to:
- Order your repeat prescriptions
- Make an appointment
- Cancel an appointment
- Change your contact details
- Review your summary record, medications and known allergies
- View your test results (subject to the GP’s discretion)
